Wyoming

Please check the list of licenses UBH/Optum accepts in the state of Wyoming.
You must hold an unrestricted, valid license from this list for consideration to join the Optum Network.


Optum recognizes that Wyoming is an Any Willing Provider state and therefore accepts applications in compliance with this legislation.

The following counties reflect the current need for providers based on Optum's existing provider network and membership:

MDs, DOs, PAs, RNs with Prescriptive Authority

  • In all counties

MDs, DOs, RNs, PhDs and MA Providers that provide any of the below specialties:

  • Child and Adolescent Services
  • Nursing Home Consultation
  • In-Home Services
  • Targeted Case Management
  • Intensive Case Management
  • Community Mental Health Centers

Update: CAQH Participation is required for credentialing in this State. If you do not have a CAQH ID #, please go to www.CAQH.org to register and obtain the required number before starting your credentialing application.
