Join Our Clinician Network - Florida

Please check the list of licenses Optum accepts in the state of Florida. You must hold an independent, unrestricted and valid license from this list for consideration to join the Optum/OHBS-CA Network.

GROUP CONTRACT: Group contracts are available under limited circumstances. If your group is a hospital-based or public-based practice or a group practice with five (5) or more providers that share the same Tax ID and have a centralized intake and billing department, please fax your request for Group Contract consideration to your state Network Management team. The fax number is located on the Contact Us page under the Network Management Contact Information section.

Reminder: If you do not fall under one of the recruitment categories shown on your state network page, Optum/OHBS-CA is unable to accommodate your request at this time. The state network page is reviewed and updated according to business and geographic need. Please check the web site periodically for changes.

TO SUBMIT A REQUEST FOR OPTUM NETWORK PARTICIPATION, COMPLETE AND SUBMIT THE ONLINE FORM:

1. Network Participation Request Form (NPRF)
   

   Follow the online instructions to complete and submit the Network Participation Request Form. Upon receipt, we will review the form and contact you regarding the next steps in the application and credentialing process.

   We think you’ll find the online form very straightforward and convenient. If you don’t finish it in one sitting, you can easily return to it and complete at a later time using the same link as above. However, if you encounter any difficulty in accessing or completing the form please email us at:  se.region.optumpr@optum.com and we'll try to address your online submission issues.
