Submit ABA Prior Authorizations Online via a new, Faster Process
There’s a new way to submit prior authorization requests for Applied Behavior Analysis (ABA) services – and it’s a real timesaver for you.
Behavioral health providers must now use the new Massachusetts ABA Assessment and Treatment Plan form when requesting authorization for members covered by a Commercial health plan.* Using a standardized form is required by the Massachusetts Division of Insurance.
What you need to do
The new standardized form is faster and easier than submitting requests via email or fax. Just follow these steps:
- Complete the form – be sure to fill in all fields marked mandatory
- Submit the form – you should see a confirmation page after the successful completion
Note: Only the licensed Applied Behavioral Analyst (LABA) rendering and/or supervising the autism services should complete the form.

*Submitting the Massachusetts ABA Assessment and Treatment Plan form doesn’t guarantee authorization of your request. Use of the form is required for all authorization requests submitted on or after July 17, 2024.
