You must hold an unrestricted, valid license from this list for consideration to join the Optum Network.

We are looking to recruit facility based services identified below in Tennessee. If your facility provides one of these types of services please use the Optum Facility Network Request Form (FNRF) and follow the instructions to send the completed FNRF to Network Management.

  • Home Health Agencies (psychiatric nurses and mental health counselors)
  • Partial Hospitalization Programs (particularly child/adolescent)
  • IOP Programs

Currently, Optum may be accepting requests for network participation in these geographic areas. If your practice is not located in a county/area listed below, Optum/OHBSC is not able to accommodate your request for network participation at this time.

MDs, DOs, PAs, RNs with Prescriptive Authority

MDs, DOs, RNs, PhDs and MA Providers with any of the below specialties:

  • Nursing Home Consultation
  • In-Home Services
  • Targeted Case Management
  • Intensive Case Management
  • Community Mental Health Centers
