Can I submit my EAP claims electronically?
Yes. EAP claims can be submitted here on Provider Express by using Claim Entry and choosing the EAP option. Simply log in to begin.

Note: EAP claims can be submitted at no cost to you through Provider Express secure transactions. EAP claims should be submitted through Provider Express or on a CMS-1500 claim form using the standard "HJ" modifer in section 24D.
